Health and Wellness

All students are required by the Vermont Department of Health to have a completed immunization history on file in the VTSU Wellness Center. A completed health form along with your most recent physical signed by your primary care provider is also mandatory. These records MUST be on file at the Wellness Center before you attend your first day of classes. Forms can be accessed and submitted on the student health portal

You should have your forms completed and up to date in order for the staff at the Wellness Center to help you medically.
